Verifications

There are several ways to verify the status of an Iowa physician's or acupuncturist's license.

Written Verification

To a State Licensing Board

Verification to a U.S. medical or osteopathic licensing board is processed using VeriDoc. Go to www.veridoc.org to request the verification of your Iowa license to another licensing board.  You will pay by credit card and the verification will be sent immediately to the state medical or osteopathic board. Each request will include verification of all licenses a physician has held in Iowa, e.g., resident and permanent licenses.

Please note that ALL states accept VeriDoc verifications. The board will not complete other state medical board license verification forms. The Iowa board's standard verification letter generated through VeriDoc will provide all information required by a state medical board.

To an Agency Outside the U.S.

For requests to agencies outside the U.S. (Canada and overseas), sign in to your Online Services account and select 'Verification of physician license to state agency' MD or DO. 

To Another Organization

Verification of a license that does not require a certified statement or letter is $15 per license.   Submit a written request indicating the license that needs to be verified and where the verification is to be sent, along with the appropriate fee. Mail the request to the Iowa Board of Medicine, 400 SW 8th Street, Suite C, Des Moines, IA 50309-4686. Make checks payable to the Iowa Board of Medicine.

 

Find a Physician or Acupuncturist

Find a Physician or Acupuncturist is a system that allows you to access the board's database via the Internet. The information is accessible 24 hours a day, and is available by name and license number. Information on Find a Physician or Acupuncturist is updated daily and the service is free to everyone.
