Change of Address
Address changes must be completed by the licensee. Board rules require all licensees to inform the board of any address change within thirty (30) days of the move.
Go to the Online Services website to update an address or add additional work address locations (instructions below).
If you wish to inform us of a name change, you will need to do so in writing with supporting documentation.
Send us a letter signed by the licensee documenting the previous name and now your current legal name along with a notarized copy of your legal name change document(s).
**Documents will not be returned to the licensee. DO NOT send the board your original paperwork.**

It is imperative the Iowa Board of Medicine maintains accurate contact information for all licensees in order to ensure renewal notices and other important correspondence are sent to an appropriate address.
