MD and DO Number Formatting

MD licenses are a 5 digit number - i.e. 12345.

DO licenses are a 4 or 5 digit number.

• If the license number is less than 3261 add 0's in front of the license to make a 5 digit number .
  • If your license number is 3260 enter 03260.
  • If your license number is 213 enter 00213.
• If the license number is equal to or greater than 3261, enter the 4 digit number.
  • If your license number is 3333 enter 3333.
